A man falls into the water while fishing near the shore. He can swim, but whenever he tries to surface he repeatedly hits a hard surface overhead. Rescuers find an opening only where he fell in.

The answer
He had been fishing on a frozen lake. After falling he drifted beneath the ice and lost the entry point. Being near the shore did not give him air; he needed to find the opening because the ice prevented him surfacing elsewhere.
